This trip is open to high school youth currently in grades 9-12.

Volunteer Information Form (all participants) – may be completed and returned electronically
Parental Consent Form (Youth Participants) – must be signed and notarized before returning to church
Adult Participant Release (Adult Participants) – print, sign and return to church
